Hip sprains are less common than sprains in joints like the ankle or knee, partly because the hip is an inherently stable joint. The ball and socket structure of the hip, combined with strong surrounding ligaments and muscles, means it usually takes significant force to seriously destabilise it.

Why Hip Dislocations Are Rare
The hip joint is designed for both movement and stability. The socket of the pelvis is relatively deep, and the labrum — a ring of cartilage around the edge of the socket — helps deepen it further.
When a hip sprain does occur, it usually involves overstretching or repeatedly irritating the ligaments. A true dislocation would mean the “ball” part of the joint would need to pop out of a socket with a smaller opening than the widest part of the ball. Unless, of course, the socket is wider than the ball. This can be due to an unusually shallow socket, or a high impact trauma, like a road traffic accident.
For most people, however, a hip sprain is much more likely than a true dislocation.
How Hip Sprains Happen
Hip sprains often occur during sudden twisting, overstretching, awkward landings, or sporting collisions. Football, dance, martial arts, gymnastics, and running sports can all place significant stress through the hip joint. Symptoms may come on suddenly after an obvious movement, or develop over a few days.
Common symptoms include:
- pain deep in the hip or groin
- pain on walking or climbing stairs
- stiffness and aching, especially after rest
- pain on twisting movements
- tenderness around the joint
Because the hip is surrounded by multiple strong muscles, patients sometimes initially assume they have strained a muscle rather than irritated the joint itself. Especially when pain from the hip joint often refers to the groin.
Hip Sprains vs Snapping Hip
Hip pain is not always caused by ligament injury. Snapping Hip Syndrome can also produce discomfort and unusual sensations around the joint.
In snapping hip syndrome, tendons move over bony points around the hip, making a snapping or clicking sensation during movement. This condition commonly affects dancers, runners, and gymnasts. Unlike a sprain, snapping hip may not involve a clear injury at all. Symptoms are often more movement-specific and may occur without significant instability or swelling. Some cases are more due to tension in the tendon, coming from a tight muscle, than an injury as such.
That said, repetitive snapping can sometimes irritate surrounding tissues and contribute to secondary pain.
The Role of Labral Tears
Some symptoms associated with hip sprains overlap with labral injuries. The labrum helps stabilise the joint and improve load distribution. Tears can develop through sudden trauma or repetitive rotational stress. Therefore, twisting or squatting movements are common aggravating factors. There might also be stiffness after periods of sitting or rest, and sensations in the groin or hip joint. Catching, locking, and clicking may all indicate a labrum injury.
Because these symptoms overlap with several other hip conditions, assessment is important to determine what structures are most involved.
Osteopathy for Hip Sprains
Treatment focuses not only on the painful hip itself, but also on the wider mechanical picture. Reduced movement through the lower back, pelvis, or thoracic spine can all alter loading through the hips. It is also common for compensation patterns to develop after injury, particularly if walking mechanics change. It’s possible that a sprain was predisposed by increased demands coming from a problem elsewhere, too.
Activity modification is often important during recovery. Completely avoiding movement is rarely ideal, but repeatedly aggravating the tissues can delay healing.
